Richard Hoffman


rich hoffman baseball cardRichard Hoffman is the owner and manager of multiple physical therapy practices based in Florida, including Superior Rehab Systems, and Med Diagnostic Rehab of South Florida. A former pitcher with the St. Louis Cardinals organization, Mr. Hoffman sustained a shoulder injury, wherein he had his first encounter with Physical Therapy. He found the field very rewarding and became a licensed therapist himself.

In addition to his therapy degree from Miami Dade, Mr. Hoffman has a Bachelor’s degree in Management and Marketing with a minor in Finance from Long Island University, Brooklyn, New York and a Masters degree in Health Science Administration from Nova Southeastern University, Davie, Florida and a second Masters degree in Management from Harvard University, Cambridge, Massachusetts.

Mr. Hoffman specializes in outpatient therapy development, revenue cycle management and has extensive rehab background. He has been directly involved with over 70 CORF’s, ORF’s and Part B projects throughout the country, from initial certification to continual practice management.

David Szmiga, DPT

Doctor of Physical Therapy

Dave began his professional education at the University of Florida in which he graduated with a Bachelors of Science with a Specialization in Athletic Training. He became certified as an Athletic Trainer and a Strength and Conditioning Specialist. At the University of Florida, he had the wonderful experience working with the Florida Marlins Baseball Team, University Athletics, high school and college athletes, and a physical therapy clinic.

Dave decided to further his education at the University of St. Augustine where he graduated with a Doctorate in Physical Therapy. The University of St. Augustine specializes in Orthopedics and Manual Therapy with an emphasis on Assessment and Treatment of the spine. At the University of St. Augustine, Dave also met his beautiful wife, Loren. After graduation, Dave returned to South Florida where he was born and raised to begin his career with MedDiagnostics Rehab in the Delray office.

After about one year of working, he returned to the University of St. Augustine to further his knowledge and take the extensive testing of the Manual Therapy Certification. He never stops his learning in order to help as many people as possible. He took additional courses to become Pilates Certified and a Performance Enhancement Specialist.

Dave's goal as a Regional Manager and Physical Therapist is to provide optimal patient care in a professional, friendly, and fun environment. The highly trained and caring staff at his clinic are all wiling to do whatever is necessary to help you achieve your wellness goals.

pam brosky

Pamela Brosky, MPT

Master of Physical Therapy

Pam is a licensed MPT with a Masters degree from the University of St. Augustine with emphasis on manual therapy and spinal manipulation and stabilization. Pam has over 11 years of experience in orthopedics and is currently the regional manager in Broward County.

Born in Allentown, PA, she moved to Fort Lauderdale for her love of the outdoors year round. Pam chose the profession of physical therapy because of her love of sports and her true understanding of the gift of one's health.

In the clinic, Pam's patients range from young athletes to the active senior population. She sees musculoskeletal and neurological dysfunctions along with gait disturbance and balance and vestibular disorders.

When not working as a therapist, Pam enjoys running on the beach, gardening, hiking, and traveling.

It is Pam's goal at MedDiagnostics Rehab for patients to reach their maximum potential with our compassion and expertise in our field.

Claudia Coba, DPT

Doctor of Physical Therapy

Claudia Coba first knew she wanted to be a physical therapist when she was 16 and volunteering at Plantation General Hospital. She loved the sense of accomplishment and the camaraderie between the patients and the therapists. Claudia then went on to get her Bachelors in Life Science, Master in Physical Therapy, and Doctorate in Physical Therapy at Nova Southeastern University. She is presently working in an orthopaedic based clinic but has also worked with neurological patients and balance impaired patients. Ms Coba was born and raised in South Florida with parents from Ecuador making her bilingual in English and Spanish.

Martin Castillo, DPT

Doctor of Physical Therapy

Martin is originally from Queens, New York and has been living in South Florida for the past 31 years. He is fluent is Spanish. Over the years Martin has been active in various sports such as Judo, Aikido, baseball, and soccer. For the last 18 years he has been an active coach for boys and girls soccer teams for various city leagues.

Martin has been practicing Physical Therapy for the last 31 years. The last 25 years he has devoted to orthopedics working very closely with orthopedic surgeons in the management of acute to chronic conditions. He has been invited by numerous surgeons to be along side them and observe various surgical procedures giving him the unique experience of observing and discussing the procedures directly with the surgeon. Martin has also owned and operated his own P.T. services company and managed various outpatient facilities. Currently Martin is using his experience at MedDiagnostic Rehab to help the company fulfill the patients' needs and goals to the best of their ability.

David Adames, PTA, LMT

Licensed Physical Therapy Assistant

David was born in the New York City and lived mainly in Jamaica, Queens until his move to Florida in 1994 to work in a hospital as a computer programmer. Eventually he left the computer field to pursue a new career in Physical Therapy.   David attended South University, graduating with honors.  His primary interest is in manual therapy techniques as he is also a licensed massage therapist.  David is fluent in Spanish and enjoys playing music as well as dancing salsa and merengue.  He and his wife Joanne are doting grandparents to little Sophia.

Michelle Mercader, PTA

Licensed Physical Therapy Assistant

Michelle is a native Floridian. She received her Bachelors of Science degree from Florida Atlantic University in 2000. She graduated from Broward Community College in 2003 as a Certified Physical Therapist Assistant. She believes in a hands on approach with all patients and treating each patient as an individual to enhance their quality of life and independence. Her special interests include manual therapy, joint and soft tissue mobilization, and myofascial techniques.

Michelle's hobbies include working out, playing pool, and enjoying Florida's beautiful beaches.

avi bregman

Avi Bregman, MPT

Master of Physical Therapy

Avi is a Licensed Physical Therapist at MDR Rehab’s Delray location. He grew up in Ottawa, Canada and received his education at The University of Western Ontario in London, Canada where he completed his Bachelor of Science Degree in Kinesiology. He has worked and volunteered as a Certified Kinesiologist for both orthopedic and neurological Physical Therapy clinics.

Avi later moved on to completing his Master of Physical Therapy Degree at The University of Western Ontario. He has also taken additional specialization courses in the areas of Wound Management, Manual Therapy, Acupuncture, and has worked in the field of golf rehabilitation.

Avi is a dedicated therapist with the goal of providing exceptional quality of care and achieving superior patient outcomes. His passion outside of physical therapy is golf which he used to play competitively and continues to play leisurely.

Steve has been a therapist at Superior Rehab for the past five years. He loves the one-on-one interaction with his patients and the "family atmosphere", where the therapists, receptionists, owners, and patients all know one another and are on a first name basis. He feels confident that anyone coming to our facility will find professionalism, knowledge, and understanding of any challenges that the patients may face. Steve loves his career as a therapist because he is a loving, caring person that loves caring for others.

Valentyna Nesterova, DPT, certified CI

Dr. Nesterova was born and raised in Ukraine, Kharkov city, where she got her degree in Russian literature, language and management. She moved with her husband to US in June 1999. While in New York she was a manager in PT office, and realized that she loves to work with people and decided to become a physical therapist. She graduated from NSU in 2007 with doctorate degree in physical therapy (academic excellence award 2005, 2006, 2007; class clown award). After graduation, she worked in many different rehab facilities such as hospitals, SNF, pediatrics, but her passion always being outpatient rehab. Dr. Nesterova enjoys working with different patients such as ortho, neuro, athletes, pediatrics; she believes that each patient is unique and needs individual attention and treatment plan. Her work days are fulfilled with joy, love and new learning experience and knowledge. She believes that learning is an ongoing process that hopefully will never end.

Joseph Bernreuter DPT, NASM PT

Doctor of Physical Therapy

Born and raised in South Florida. Having an older sister with Cerebral Palsy, Joe developed an early interest in Physical Therapy. Joe attended St. Thomas Aquinas High School in Fort Lauderdale. Through his teenage years Joe went through his own bouts of Physical Therapy for shoulder surgeries. Having first hand experience of the service that Physical Therapists provide furthered his interest in the field. With this motivation Joe attended Florida State University where he received a Bachelor's Degree in Exercise Physiology and earned multiple Dean's List awards for exceptional performance in the classroom. To continue pursuing his his dream Joe chose to attend Florida International University where he would receive his Doctorate of Physical Therapy and multiple awards for class performance. Joe has also been a personal trainer for more than 4 years and truly loves helping people improve their lives through rehab and physical fitness.

Physical Therapy Assistant

Samara was born in New York, and has lived in South Florida for the past 27 years. Samara's passion in life has always been helping others which led her to a career in Physical Therapy after graduating with academic honors from Lynn University with an associate of Science degree in 1996. Samara has spent the last 15 years specializing in orthopedic rehab care as well as her own personal health and fitness. She also has clinical experience with acute care, wound care, pediatrics, geriatrics, and Neurological disorders. Samara understands the importance of living an active and healthy lifestyle which is why she always strives to improve the quality of life for her patients while maintaining a gentleness and empathetic demeanor.

Samara enjoys being a dedicated mother and loving wife. She also likes to work out at the gym and spend time traveling with her family.

Sheri Wolf, PTA

Physical Therapy Assistant

Sheri Wolf was born and raised in Southern California and moved to Florida in 1981. She graduated from High School in 1987, but decided to go back to school in 2002 after being married for 12 years and having 3 daughters. In 2005 she received her Associate in Science Degree for Physical Therapist Assistant and graduated with highest honors. Since graduating she has been in the outpatient field working with orthopedic and neurological patients. Sheri has been with Med Diagnostic Rehab since April of 2006.

Desirée DiClemente

Receptionist/Therapy Tech

Desirée DiClemente was born and raised in Fort Lauderdale. As a three time state Champion Swimmer from St. Thomas Aquinas High school she knows what it is like to be part of a team working toward a goal of excellence. She started working for Superior Rehab in 2007, after returning back to South Florida from attending the University of South Florida in Tampa. Recently Ms. DiClemente has enrolled in the P.T.A. program at Broward College after greatly enjoying her experience in the physical therapy field. She works as a receptionist and therapy tech at the Coconut Creek location. Desirée enjoys the hands on friendly atmosphere involved in the clinical setting. She enjoys listening to the patients' stories and helping them on their road to recovery. As a long time Miami Dolphin's fan Ms. DiClemente love's going to football games. She also enjoys fishing, boating, spending time with her friends, family, and her dog Big.

Nicole Kleps


Nicole Kleps is a friendly, proactive, high-energy receptionist and Physical Therapy Tech who puts patients at ease as soon as they enter the east Boca Raton orthopedic clinic. She says her specialty is encouraging patients’ participation in physical therapy and helping them take their minds off their pain.

While attending Coconut Creek High School, Nicole actively participated in Health Occupations Students of America. Not only did she gain important fundamental knowledge about health, disease, and the human body, but her initiative and sense of responsibility led to Nicole becoming treasurer for the organization and its Alumni group.

Nicole has worked in the health care field since graduation with a variety of providers in the areas of podiatry, orthopedics, and physical therapy. Nicole enjoys playing softball, spending time with family and friends, and traveling.

Lisa Simpson


Lisa has been working in orthopaedics since 1998 and has been with MDR since 2006. She is a medical assistant and OrthoTech. She finds being in this field rewarding and exciting and enjoys helping out in the gym and seeing the patient's progress.

She is married with two awesome children. Her hobbies include arts-and-crafts, reading, and spending time with friends and family.
